To receive your Course Certificate and/or CEU Course Certificate, you must complete all eight (8) modules, pass the course quiz with 75% proficiency, and complete the course evaluation below.

Once you have passed the quiz and completed the course evaluation, email your license number and completed course evaluation to hello@oncopelvicpt.com. You will receive an email with the link to download your Course Certificate and/or CEU Course Certificate within two (2) business days of submission.
